     Re:  Fire Hazard 

     I inserted the bread pan incorrectly in my bread machine on two 
     separate occaisions and the dough failed to mix and baked as a pile of 
     ingredients.  There was not a fire but there was a mess.

     I have also heard of persons just dumping the mix into the pan and 
     starting.  There also were no fires, just a hot pile of flour and 
     other ingredients.
