I have had excellent results from a King Arthur 
(www.kingarthurflour.com) starter that I ordered eight years ago. It 
still is alive and healthy. At that time, the starter came in 
powdered form. It now comes in fresh, ready-to-use form. The catalog 
does not indicate whether it can be shipped outside the USA, however. 
Complete instructions for use are included with the starter.
